Ending of the Reaper War
Although Commander Desmond Shepard's forces had reached the beam to the Citadel, but it was Commander Alistair Creed who decided to make the run for the beam since he believed he had nothing to live for. Since Alistair chose to destroy the Reapers, this also resulted in the destruction of all synthetic life. Once Activated, the Crucible fired a pulse into the mass relay network, spreading the blast across the galaxy. This pulse damaged every relay and the Citadel in the process. Some time later, the galaxy eventually finished the repairs to the relays and recovers from the destruction the Reapers had caused.

Cmdr. Desmond Shepard rallied the galaxy to fight against the Reapers, but was saved by Commander Alistair Creed. Years later, the SSV Discovery Star (a ship he was on) hit a wormhole that ported him over.
Olo Shan lost his friend and because Commander Alistair Creed told him to accompany Shepard, he did and because he boarded the SSV Discovery Star, he too went through the wormhole and ended up in the LivVerse.
Kat Parker is the daughter of Carmen and Olivia Creed and because of her, the Creed Family had been hunted and killed by Cerberus and her Grandmother Ophelia. With nothing left to lose, she found a way to leave and cross over to the Prime Universe.
Seth Birchall is the son of Kat Parker and Dr. Sam Birchall and came over with his father when he was a toddler to be raised by his father and step-mother, Dr. Rose Birchall.
Dr. Sam Birchall is the ex-husband of Kat Parker. He'd supposedly been killed by Kat, and it was Charles Turner who nursed him back to health. He found Seth and opened up a portal to the Prime Universe to find Kat.
